Athletics Registration
All Valley Catholic High School parents must register their student-athlete online through our new FinalForms athletic registration platform. All athletes must have a current physical examination on an OSAA form on file with the athletic department. The fee for each sport is $200. Fees are not collected during registration. Your FinalForms account will be billed once rosters have been set. The maximum fee charged per student for the school year is $400.
Physicals
Physicals are required every two years and must be completed before the official start of each sport’s season. Physicals are valid for two years from the date they are issued. The physicals must be completed on the OSAA-sanctioned form and be on file in the Valley Catholic High School Athletic Office before the first day of practice.
FinalForms physicals can be uploaded to your FinalForms account, emailed to Kelly Alfieri or mailed to the school.

Transfer Students
Students transferring from another high school – not entering as freshmen – must complete an OSAA Eligible Student Transfer Certificate. This certificate must be turned in to the Athletic Office to be on file prior to participation in athletics.
